Introduction. Southern Blossom Pediatric Speech Therapy, LLC (“Southern Blossom Speech”, “we”, “us”, or “our”) operates the website www.southernblossomspeech.com (the “Site”). Through the Site, we provide information to patients, prospective patients, and members of the public. This Online Privacy Policy (“Privacy Policy’) explains Southern Blossom Speech’s privacy practices that apply to the use of the Site.
Please read this Privacy Policy carefully so that you can understand how Southern Blossom Speech collects, uses, and discloses information from and/or about you when you use the Site. This Privacy Policy is a part of our Website Terms of Use. By accepting the Terms of Use, you are consenting to the use and disclosure of your personal information as outlined in this Privacy Policy.

In addition to these privacy protections, Southern Blossom Speech is required by the Health Insurance Portability and Accountability Act (“HIPAA”) to describe its privacy practices to patients in a document called a Notice of Privacy Practices. You will be provided a copy of this document when you begin using our services (the “Services”).
Third Party Sites. The Site may contain links to other websites. Southern Blossom Speech does not share personal information with those websites. Southern Blossom Speech is not responsible for the privacy policies of any site linked to this Site and not wholly owned and controlled by Southern Blossom Speech. De-Identified Information. We may use De-Identified Information created by us without restriction.
Security of Information Collected. We use industry standard physical, technical and administrative security measures and safeguards to protect the confidentiality and security of your Personal Information. However, since the Internet is not a 100% secure environment, we cannot guarantee, ensure, or warrant the security of any information you transmit to us. Supplemental Privacy Policy For California Residents

This Supplemental Privacy Policy for California Residents (“Supplemental Privacy Policy”) supplements and is expressly made part of the information contained in Southern Blossom Pediatric Speech Therapy, LLC’s Online Privacy Policy, and it applies solely to the visitors to our websites, our patients, and others who reside in the State of California (“consumers” or “you”). We adopt this Supplemental Privacy Policy to comply with the California Consumer Privacy Act of 2018 (“CCPA”).
For purposes of this Supplemental Privacy Policy, “Personal Information” means information that identifies, relates to, describes, is capable of being associated with or could reasonably be linked, directly or indirectly, with a particular consumer, household, or device.
If you are a California resident who has provided Personal Information to Southern Blossom Pediatric Speech Therapy, LLC (“Southern Blossom Speech”) via the Site or a California resident who believes Southern Blossom Speech collected your Personal Information, you may exercise your rights under CCPA as explained in this Supplemental Privacy Policy.
RIGHT TO KNOW
Subject to certain limits, you may ask us to provide you with the following information for the 12-month period preceding your request:

Note: You may make no more than two requests for information in a 12-month period.
RIGHT TO REQUEST DELETION
You may also ask us to delete any Personal Information that we have collected from you. Under the CCPA, we are not required to delete Personal Information that is necessary to: provide our services; detect security incidents; protect against malicious, deceptive, fraudulent, or illegal activity; debug to identify and repair errors that impair our sites and/or services; enable solely internal uses that are reasonably aligned with your expectations based on your relationship with us; comply with a legal obligation; or use Personal Information internally, in a lawful manner that is compatible with the context in which you provided it.
We do NOT sell any personal information collected from website visitors.
RIGHT TO NONDISCRIMINATION
We will NOT discriminate against you for exercising your rights under the CCPA, including by:

Charging you different rates for services, including through granting discounts or other benefits or through imposing penalties;
Providing you a different level or quality of services; or
Suggesting that you may receive a different price or rate for services.
However, the CCPA permits us to offer you certain financial incentives that can result in different prices, rates, or quality levels, which are related to the value of your Personal Information. Participation in a financial incentive program requires your prior opt-in consent, which you may revoke at any time.
SUBMISSION OF REQUESTS TO KNOW AND/OR REQUESTS TO DELETE
You may submit a request to know or a request to delete by submitting an email request to andree@southernblossomspeech.com.
We are required to verify your identity before responding to a request to know or a request to delete.
Please note that following your verified request, we will send you your Personal Information. Any response to your request, including any Personal Information, may be sent as an encrypted file.
Generally, we will process and respond to your request within 45 days. As permitted by the CCPA, if we need more time to respond, we will tell you why and our time to respond will be extended by an additional 45 days (up to 90 days total).
Once you have submitted a request, we will send you a receipt acknowledging your request within 10 days. Please be sure to check your email spam folder or filter for this receipt. If you do not receive such a receipt within 10 days of your submitted request, please send an email to us at andree@southernblossomspeech.com because an error may have occurred.
You also have the right to designate an agent to exercise these rights on your behalf. We may require proof that you have designated an agent to exercise these rights on your behalf and to verify your identity directly with us. HOW WE SHARE PERSONAL INFORMATION
Southern Blossom Speech may disclose your Personal Information to third parties as described in our Privacy Policy and for certain other business purposes. When we disclose Personal Information for a business purpose, we enter a contract that describes the purpose and requires the recipient to both keep that Personal Information confidential and not use it for any purpose except as permitted under the contract.

California Civil Code Section 1798.83, you have the right to request us to disclose the identity of any third parties to whom we have disclosed Personal Information within the previous calendar year, along with the type of Personal Information we disclosed, for the third parties’ direct marketing purposes. Please note that we are only required to respond to a customer request once during any calendar year and only if we are covered by such California Civil Code section. Please be aware that not all information sharing is covered by this California law and only information sharing that is covered will be included in our response. CHANGES TO THE SUPPLEMENTAL PRIVACY POLICY
Southern Blossom Speech reserves the right to amend this Supplemental Privacy Policy at our discretion at any time. When we make changes to this Supplemental Privacy Policy, we will post the updated Supplemental Privacy Policy on the websites and update the Supplemental Privacy Policy’s effective date. Your Southern Blossom Speech use of our websites following the posting of changes constitutes your acceptance of such changes.
